PHIMOSIS

Phimosis is the condition of the penis characterized by a narrow foreskin which cannot be retracted over the glans penis. It may be primary (physiological), i.e. congenital, which is a condition patients suffer form since birth. However, in most cases this condition is temporary and is usually resolved until the age of three (90% of cases). Phimosis may also be defined as secondary condition, which is a condition that occurs due to repeated injury by rough foreskin retraction (balanitis obliterans xerotica).

CIRCUMCISION

Circumcision represents the surgical removal of entire or part of the foreskin from the penis. In certain number of men, the circumcision is performed for religious reasons. Circumcision may also be performed for medical reasons, i.e. in cases when the foreskin cannot be retracted over the glans penis. If there are certain difficulties in retracting the foreskin, applying a neutral lubricant (“Pavloviceva mast” cream or equivalent) and repeated “retraction” are recommended. If retracting the foreskin is extremely difficult, you should not insist on repeating it frequently since that can cause the condition, known as paraphimosis, when a part of the narrow foreskin creates a ring around the body of the penis, thus preventing the blood supply to the glans penis. In such cases, a surgical procedure is urgent. In addition, frequent and rough foreskin retraction may result in foreskin injury, bleeding, infections (balanoposthitis) and scarring. Such a condition needs to be addressed surgically as soon as possible. Precancerous lesions of the glans penis may also represent a reason for circumcision. Such a procedure reduces the risk of cancer.

Procedure

Circumcision is a surgical procedure which is usually performed under local anesthesia. Anesthetic cream is applied over the foreskin preventing the patient to feel even the sting of a needle used to administer anesthesia. The procedure takes about 30 minutes. After administering local anesthesia, when we are certain that the patient does not feel any pain, the excess foreskin is surgically removed.  It is of great importance for the surgical procedure to be performed well and for bleeding to be stopped quickly and carefully. The wound is closed by an individual seam, made using absorbable sutures. The wound healing period depends on the type of the seam. After the procedure the wound is bandaged and the patient receives instructions on how to maintain the hygiene of the penis until the suture removal.

The first follow-up medical examination is usually scheduled 24 hours after the procedure. The recovery usually lasts 7-15 days, during which the patient is advised to abstain from having sexual intercourses. No matter how tempting it may sound when the doctor says that the sutures are not removed and that they are absorbable, sometimes the patient requires some “assistance” in order to prevent ingrown stiches.

Results of circumcision. During the first several days after the procedure the sensitivity of the glans penis is heightened. Therefore the application of medical ointments over the glans penis is recommended. As time passes, the mucous membrane of the glans penis becomes less sensitive, and hardens, thus becoming less susceptible to sexually transmitted diseases. Many studies have shown that the incidence of the glans penis cancer is less common in circumcised men. Accordingly, the incidence of cervical cancer is less common in their sexual partners.

Recommendations: If you are experiencing aforementioned problems, if you are in doubt - do not postpone making an appointment with a plastic surgeon. If you are having difficulties with wound healing after the penis surgery – contact your plastic surgeon. Treatment of such conditions often does not involve surgical interventions. A detailed examination and good advice can help you solve the problem.

FRENULIM BREVE

Frenulim breve

Frenulim breve, or short frenulum, is not a rare birth defect of the male external genital organ.

Usually the patient has no pronounced complaints, but it should be kept in mind that with the presence of a short frenulum, the penis becomes shorter. Frenulotomy is a simple surgical intervention that separates the scrotum from the glans. It is performed under local anesthesia and the recovery time is several days.
